    Patient Center Medical Home Services

TLC provides a full suite of services to support the deployment of a Patient Center Medical Home.   These services enable practices to move through the process quickly and have trained NCQA CCE experts to assist in the practice transformation.   Within New York State, TLC is working with a grant that funds these services and even include a discount for the PCMH applications providing dual saving for the practice.  Here is a suite of our Practice Transformation Services

  • Provide guidance to primary care practices to achieve PCMH 2017 recognition
  • Provide examples of best of breed practices samples to follow that enables practice into highly integrated team-based care providers
  • Practices will work over time to meet 52 Core requirements and 6 Elective criteria
  • The program will take 6 – 12 months to complete depending on current PCMH status
  • Fee waiver for NYS PCMH participating practices when registering in Q-PASS
  • Assist in HIE/RHIO implementation

HIT/IT Support Services and Security Services

For small and medium-sized practices, TLC can take on the role of IT support specialists and deal with problems related to EMRs. For organizations that lack IT staff, the company provides extra assistance by working with the EMR vendors to set up the entire system. A unique attribute about TLC is that we assess risks within an organization by ensuring that they follow HIPAA and MU guidelines.
